New Patient Registrations

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. We ask that you provide two forms of ID when you register with the practice, one with a photograph, and one with your address details. This is so that we can confirm your eligibility to register with the practice.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide us with useful information whilst we wait for your medical records to arrive from your previous doctor. We do require 48 hours to process your registration. Medical treatment is available from the date of registration. Please contact reception for further information.

Print off GMS1 – New Patient Registration Form (if you are unable to print the form, simply call in and collect from reception) and complete in full.

Bring your completed registration forms along with your two forms of ID to the surgery to show the receptionist.

Coronavirus (COVID-19)

The NHS and Public Health England (PHE) are extremely well prepared for outbreaks of new infectious diseases. The NHS has put in place measures to ensure the safety of all patients and NHS staff while also ensuring services are available to the public as normal.

If you have a high temperature, new continuous cough or a loss of, or change in your normal sense of taste or smell please do not attend the surgery.
